Displaying Multiple columns in Cascading prompt/LOV

Hi,

I am trying to create a cascading prompt to display prompt selection as below
Region_ID -- 1st level
State_ID --2nd level
City_ID, City_Name --3rd level

The city name can be a person name responsible for that city. User would like to see that field for making the selection, but the cascading prompt will only take one field at each level. Please let me know if there is any other way to achieve this.
